A 50-year-old man was admitted to our hospital with minimal shortness of breath. Chest X-ray showed left lower zone infiltration and mediastinal shift to the left side. Thorax computed tomography was done and showed right lung overgrowth and left destroyed lung.

A single plueral space is called “buffalo chest” because the absence of anatomçal separation of the two hemithoraxes resembles that of the Nort American buffalo or bison. In ouur patient, the presence of a “buffalo chest”made the initial chest X-ray interpretation difficult. A possible pneumothorax should be catastropic for this patient, so he should be closely monitored regarding any invasive procedure or trauma.
