Comparison of postoperative results of laparoscopic nissen and nissen-rossetti fundoplication

Aim: Nissen fundoplication, which is performed laparoscopically, is the most frequently preferred surgical technique worldwide in the treatment of gastroesophageal reflux disease (GERD). This technique has subtypes such as Nissen and Nissen-Rossetti. This study aimed to investigate and compare the effects of Nissen and Nissen-Rossetti techniques of Nissen fundoplication, which is the most preferred technique by surgeons in the surgical treatment of GERD, on patients in the postoperative period.
Material and Methods: Patients, who were operated on for GERD with laparoscopic Nissen fundoplication (LNF) and laparoscopic Nissen-Rossetti fundoplication (LNRF) techniques in the general surgery clinic of a tertiary care hospital between January 2011 and May 2016 were divided into two groups, and preoperative, intraoperative, and postoperative findings were examined in detail, and these patients were contacted.
Results: In this study, 44 (47%) patients underwent LNF surgery, and 49 (53%) patients underwent LNRF surgery. When dyspepsia and other reflux symptoms in patients in the postoperative period were investigated, in the postoperative 4th week and postoperative 6th month, there was no significant difference between the LNF and LNRF groups.
Discussion: Although there is no obvious superiority of the two techniques over each other, the determining factor regarding the method to be chosen should be the opinion of the surgeon and the perioperative findings as a result of the evaluation made by the surgeon on the patient. It would be appropriate for the surgeon to choose the most suitable technique according to the patient.

Aim: The steep Trendelenburg position is frequently used in laparoscopy to improve the surgical image. However, this positioning combined with pneumoperitoneum raises concerns especially in relation to cerebral hemodynamic physiology.
In this study, we aimed to evaluate the effects of pneumoperitoneum and steep Trendelenburg position, applied in laparoscopic gynecological surgeries on cerebral venous return, with changes in internal jugular vein (IJV) blood flow.
Material and Methods: Twenty patients who underwent laparoscopic gynecological surgery under general anesthesia were included in the study. IJV cross-sectional area (CSA) and Doppler flow velocity were measured with an ultrasonography device at the end of inspiration. IJV blood flow (ml/min) was calculated with the formula of cross-sectional area (cm²) × Doppler flow velocity (cm/sec) × 60. The first measurement was performed in the supine position immediately after endotracheal intubation. The second measurement was performed 1 hour after applying pneumoperitoneum and 25° Trendelenburg position.
Results: In the steep Trendelenburg position, there was a significant increase in right and left IJV CSA (p = 0.001; p = 0.038) and blood flow (p = 0.005; p = 0.039), while no significant difference was observed in Doppler flow rate.
In the supine position, the right IJV CSA and blood flow were significantly greater than the left IJV CSA and blood flow (p = 0.015 and p = 0.017).
Discussion: The steep Trendelenburg position and pneumoperitoneum in laparoscopic gynecological surgeries caused an increase in bilateral IJV blood flow to raise cerebral venous drainage.

Clinical significance of mild thrombocytopenia in cesarean sections: An underestimated risk factor

Aim: Mild thrombocytopenia, a commonly overlooked condition, may significantly influence clinical outcomes during cesarean sections, with potential implications for blood transfusion needs, intravenous iron therapy, and postoperative hemoglobin levels.
Material and Methods: This retrospective cohort study was conducted on pregnant women undergoing cesarean sections at a tertiary healthcare center from January 2021 to December 2022. The participants were divided into normal platelet (n:636) and mild thrombocytopenia (n:435). Data were collected on demographic and anthropometric variables, obstetric and medical history, preoperative-postoperative laboratory values, and operative details.
Results: Postoperative complications were more prevalent in the mild thrombocytopenia group (14.3% vs. 3.5%, p=0.001). Mild thrombocytopenia was associated with higher pre- and post-treatment hemoglobin levels (12.2±1.5 g/dL vs. 11.7±1.3 g/dL, p=0.001, and 10.1±1.5 g/dL vs. 9.9±1.4 g/dL, p=0.026, respectively). The rate of individuals with a decline in hemoglobin levels of ≥3 g/dL was higher in the mild thrombocytopenia group (25.3% vs. 10.7%, p=0.001). Independent predictors of outcomes included cesarean section in the last delivery (OR 2.96, p=0.003), presence of postoperative complications (OR 0.676, p=0.012), lower preoperative hemoglobin levels (OR 0.175, p<0.001), and increased gestational week (OR 1.219, p=0.014).
Discussion: Mild thrombocytopenia was associated with higher preoperative and postoperative hemoglobin levels and its substantial drop. The evident correlation with postoperative complications and the potential influence on blood transfusion demands necessitate a paradigm shift in its clinical perception and management of cesarean sections.

Aim: Obesity and its complications are an increasing problem in developed countries. In 2011, around 500 million people worldwide suffered from obesity (body mass index (BMI) >30 kg/m2) and this figure is projected to at least double by 2030. The aim of this study was to investigate the changes in pro-inflammatory, anti-inflammatory cytokines and total antioxidant levels (TAC) in obese patients.
Material and Methods: In our study, IL-6, which is proinflammatory inteukin (IL), and anti-inflammatory IL-33 and total antioxidant levels were determined in 56 obese patients (BMI>30 kg/m2) and 50 control groups (18.5 <BMI). It is aimed to evaluate <24.9 kg/m2). IL-6 and IL-33 levels were investigated by ELISA (Enzyme-Linked Immuno Sorbent Assay) method.
Results: IL-6 and IL-33 levels were found to be significantly higher in the obese patient group compared to the control group (p<0.05). The total antioxidant level was found to be significantly lower in the obese patient group compared to the control group (p<0.001).
Discussion: Morphological and functional changes that occur in AD during the development of obesity can cause local and systemic inflammation. Released cytokines, fatty acids, free radicals and other factors affect the immunological activity of all tissues and cells. Chronic inflammation associated with obesity leads to many health complications such as cardiovascular and liver diseases.

Aim: Timely initiation of intravenous thrombolytic therapy (ITT) in ischemic stroke patients reduces long-term neurological impairment and increases the success rate of the treatment. Implementing systems where clinicians establish guideline-based multidisciplinary cooperation is also essential. This study aimed to investigate the effect of door-to-needle time (DTN) in the emergency department (ED) on the early and late clinical courses of patients in cooperation with the neurology clinic.
Material and Methods: The data of the acute ischemic stroke patients who underwent ITT between April 2018 and March 2022 in the ED with inter-clinical coordination were evaluated retrospectively. DTN time, National Health Institute Stroke Scale (NIHSS) scores at admission, 24th hour, pre-treatment, discharge, and 3rd month modified Rankin Scale scores (mRS) of the patients were recorded by the ED physician and neurologist and evaluated statistically.
Results: Of the 102 patients included in the study, 51 were male, and 51 were female. The median age was 75. Of the patients, 45.1% underwent mechanical thrombectomy. The median admission and 24-hour NIHSS scores were 11 and 8, respectively. The median door-to-CT and DTN times were 18.5 minutes and 85.5 minutes, respectively. Patients with DTN times > 60 minutes had higher admission systolic and diastolic blood pressure levels than patients with DTN times < 60 minutes (p < 0.01 and p = 0.015, respectively). Good functional outcome rates (mRS score 0-2) at discharge and at 3 months were 39.2% and 49.0%, respectively.
Discussion: This study showed that applying ITT with inter-clinical coordination to stroke patients in the ED is associated with a good early clinical outcome.

Aim: In allergic diseases, identification of the responsible allergens and determination of the clinical features of the patients are important in terms of controlling the symptoms. In this study, we aimed to evaluate the clinical characteristics of children with cat allergy.
Material and Methods: The study is a descriptive type study with a retrospective design. The clinical features of children with cat allergy who visited our Pediatric Allergy and Immunology outpatient clinic between June 2022 and June 2023 were retrospectively evaluated based on patient files. All patients aged 0-18 years who were diagnosed with cat allergy in our clinic within a one-year period were included. Gender, age, diagnosis of allergic diseases, total IgE, eosinophil values were evaluated.
Results: Of the children with cat allergy, 38.1% (n=40) had cats at home. 70.5% (n=74) of the children with cat allergy also had house dust allergy. Total IgE values were significantly higher in children who did not have cats at home compared to those who had cats at home (p=0.008). Eosinophil values were statistically similar in both groups (p>0.05). Respiratory allergies were observed in 57.5% of those who had a cat at home, whereas respiratory allergies were observed in 67.2% (n=21) of those who did not have a cat at home (p=0.318).
Discussion: Cat allergy should be evaluated by detailed history and physical examination. It should be kept in mind that children may be at high risk of developing cat allergy even if there is no cat at home.

Aim: In this study, we aimed to demonstrate the short-term clinical outcomes related to the surgical delay of elective inguinal hernia repair based on the observational records of a single center.
Material and Method: Consecutive patients scheduled for elective inguinal hernia repair between March and June 2023 were analyzed. Patients were evaluated in two different groups: early (<3 months) and late (>3 months) repair from the date of diagnosis. Follow-up findings (pre-, intra-, and post-operative) were analyzed for different time intervals to see if surgical delay led to adverse outcomes.
Results: Patients had similar baseline outcomes in terms of pre-existing conditions, surgical techniques and adverse outcomes. Sixteen patients (59.30%) underwent surgery early (<3 months) and 11 patients (40.70%) underwent surgery 3 months after diagnosis. Delay in surgery did not have a significant effect on the outcomes of the patients. Perioperative findings were similar in both groups.
Discussion: Surgical delay does not lead to an increased risk of hernia-related acute life-threatening complications in minimally symptomatic or asymptomatic patients scheduled for elective inguinal hernia repair.

Aim: This work used immunohistochemical and radiographic analysis to show how latency time affects distraction osteogenesis in animals.
Material and Methods: Eighteen rabbits had right femur drill osteotomies and external fixators. Latency duration split the rabbits into three groups. Groups 1, 2, and 3 had latency times of 1, 5, and 10 days. After the latency period, all groups had a 20-day consolidation phase. After consolidation, the animals were euthanized, and immunohistochemical, histological, and radiological studies were performed on the distraction-induced callus tissue.
Results: In radiological evaluation, there was no significant difference between Group 1 and Group 2 and Group 2 and Group 3 (p=1,000, p=0,066). Group 1 and 3 differed (p=0.018). Histopathological assessment showed no significant difference between Group 1 and Group 2 or Group 2 and Group 3 (p= 0,557, p=0,062). Group 1 and 3 differed (p=0,001). Group 3 had more osteocalcin-positive cells than Groups 1 and 2.
Discussion: Histopathological and radiographic methods showed that a 10-day distraction osteogenesis latency time in rabbit femurs is safer than 1 and 5-day periods. Human distraction osteogenesis latency must be determined by clinical trials.

Aim: Febrile seizures are the most common neurologic disorder of infants and young children. In this study, we aimed to investigate the role of hematological parameters in patients with febrile seizures, which constitute the majority of pediatric emergency department admissions and cause stress in families.
Material and Methods: This is a retrospective, case-control study consisting of a review of charts from a pediatric emergency department of a training and research hospital. Patients were divided into two groups: a study group consisting of 170 children with febrile seizures and a control group of 130 children with fever without seizures. Hematological parameters in peripheral blood samples at the time of the first evaluation were recorded from patients’ files and compared between the two groups.
Results: The major findings of the study were that neutrophil count was higher, lymphocyte count was lower, neutrophil-to-lymphocyte ratio was higher and hematocrit, mean corpuscular volume, mean platelet volume and platelet count were lower in children with febrile seizures.
Discussion: Febrile seizures are common in children under 5 years of age and constitute the majority of pediatric emergency admissions. In this study, we found that neutrophil count and platelet count were the most important protective factors associated with febrile seizures and we think that they may be reliable, easily accessible and inexpensive markers for determining the risk of febrile seizures.

Aim: This study evaluated applications due to contact with animals that occurred in a metropolitan city center and a rural district center and aimed to compare patients’ demographic and medical conditions in order to determine the risk factors for animal attacks.
Material and Methods: This study was conducted retrospectively and was multicenter. Patients who had been attacked by cats, dogs, and other animals (horses, cows, donkeys) and therefore admitted to the emergency department for the first time were included in this study. The obtained data from the city center and the district center were compared.
Results: In the comparison of the data from the two centers, the findings showed that while the rate of female patients (52.69%) was higher in the city center, the majority of the patients who applied to the emergency department in the district center were male (69.92%) (p<0.00). It was observed that lower extremity injuries (35.91%) were more common in the city center in terms of the injured area, and biting injuries were higher (64.09%) regarding injury type.
Discussion: As in our country, in countries where the number of stray animals is high, animal vaccinations cannot be administered fully and adequately. Algorithms have been developed to create a common approach to animal bites in our country. However, different approaches and treatment protocols can be encountered in the world. These differences may be due to geographical, sociocultural, and socioeconomic reasons. Therefore, regarding animal contact identifying at-risk groups may contribute to reducing animal attacks. In this study, to our knowledge, for the first time, the risk factors for exposure to animal attacks in rural and urban areas were investigated in the literature.

Aim: In this study, we aimed to assess long-term alterations in the corneal epithelium and the ocular surface tests of patients with Behcet disease (BD) and compare the results with those of healthy controls.
Material and Methods: Forty-five patients with BD and 43 healthy subjects were included in this retrospective case-control study. Corneal epithelial thickness (CET) mapping was measured with anterior segment optical coherence tomography (AS-OCT). Noninvasive tear film break-up time (NIBUT) and meibography were measured with corneal topography. Schirmer 1 test, ocular surface disease index (OSDI), tear film break-up time (TBUT) and ocular surface staining score were reviewed. All patients had non-ocular BD. All parameters were re-evaluated annually in the 1st, 2nd and 3rd years of control.
Results: The CET was significantly thicker in the central, inferonasal, inferior and inferotemporal quadrants in the BD group compared with healthy controls (p=0,03, p=0,01, p=0,0001, p=0,001, respectively). The mean TBUT, NIBUT were shorter and the mean OSDI score was higher in the BD group compared to the control group, significantly (p=0,021, p<0,0001, p<0,0001, respectively). Meibography scores showed higher drop-out rates in the inferior eyelid in the BD group (p=0,003). Dry eye tests were improved in Bahçet’s patients with dry eye disease after treatment of DED. The difference between the baseline values and values of three consecutive years were statistically significants.
Discussion: Ocular surface changes and DED are more common in BD. Ocular surface and dry eye disease should be carefully evaluated and treated in BD patients to avoid untreatable complications of the cornea.

Aim: In this study, it was aimed to investigate the role of tenascin-C and hepcidin levels in the diagnosis of preterm premature rupture of membranes (PPROM).
Material and Methods: This study is a prospective cohort study conducted at Van Yuzuncu Yil University (YYU), Dursun Odabası Medical Center Gynecology and Obstetrics outpatient clinic between 31.03.2021 and 15.03.2022. One hundred patients aged 18 to 45 years diagnosed with PPROM at 24 to 37 weeks of gestation were included in the study as the “case group”, and 100 healthy pregnant women were included as a “control group”. Demographic data and laboratory parameters of the patients, such as C-reactive protein (CRP), leukocyte, neutrophil, hepcidin, and tenascin-C levels, were recorded.
Results: Compared to the control group, CRP, leukocytes, and neutrophil levels were statistically significantly higher in the PPROM group (p<0.05). Hepcidin value was statistically significantly lower in the PPROM group compared to the control group (p = 0.003); tenascin-C levels between the two groups were not statistically different (p = 0.161).
Discussion: Hepcidin levels can be used in the diagnosis of PPROM. Multicenter randomized controlled studies with more cases and parameters are needed.

Aim: The study planned to determine the attitudes of parents with children aged 12-17 years towards vaccinating their children against COVID-19 and the relationship between these attitudes and COVID-19 vaccine literacy, perception of control of COVID-19, perception of causes of COVID-19.
Material and Methods: A descriptive and cross-sectional design was implemented. Between April 2022 and September 2022, 259 parents participated in the study. Data were collected using the Attitudes Towards the COVID-19 Vaccine Scale, COVID-19 Vaccine Literacy Scale, Perception of Control of COVID-19 Scale, Perception of Causes of COVID-19 and the Information Form.
Results: It was found that 44.4% of the parents had vaccinated their children against COVID-19, while 55.6% had not. A positive and significant relationship was found between the mean score of the Attitudes Towards the COVID-19 Vaccine Scale and the mean score of the COVID-19 Vaccine Literacy Scale. A positive and significant relationship was found between the mean score of the Attitudes Towards the COVID-19 Vaccine Scale and the mean score of the Perception of Control of COVID-19 Scale and the macro control subdimension. A negative and significant relationship was found between the mean score of the COVID-19 Vaccine Literacy Scale and the mean score of the Perception of Causes of COVID-19 Scale and the faith subdimension.
Discussion: Parental confidence, misconceptions due to lack of knowledge and social measures defined by health authorities were effective in influencing vaccine acceptance. It was noteworthy that as vaccine literacy increased, both the reduction in misconceptions and the positive effect on families’ vaccination attitudes.

Aim: Extrapulmonary neuroendocrine carcinomas are rare tumors and clinical trials for their treatment are limited. In this study, we aimed to determine the impact of clinicopathological and prognostic factors on platinum response in patients diagnosed with advanced EP-NECs.
Material and Methods: Patients diagnosed with extrapulmonary neuroendocrine carcinoma were included in the study. General characteristics and response rates were saved. Univariate and multivariate analyses were used to determine prognostic factors. We divided patients into three groups: pancreatic, gastrointestinal and other extrapulmonary primary. Progression-free survivals were compared to each other.
Results: We analyzed 37 patients in our study. The median age was 60 in our study. The most frequent primary origin of neuroendocrine carcinoma was the pancreas. Liver metastases were correlated with poor progression-free survival. Overall response rate was achieved in 26 patients (70%). PFS was 6 months in all groups and 4 for gastrointestinal, 8 for pancreatic, 9 months for other extrapulmonary NECs (p=0.37).
Discussion: Due to the rare incidence of the disease, few randomized controlled trials have been conducted. These treatment recommendations are extrapolated from small cell lung cancer trials. Most of the patients diagnosed with advanced extrapulmonary neuroendocrine carcinomas responded to platinum-based therapy. Liver metastasis was a poor prognostic factor for progression-free survival.

Aim: The aim of this study is to investigate the presence of SARS-CoV-2 RNA genome in atherosclerotic plaques in patients with a history of COVID-19 who are scheduled for surgery due to peripheral artery disease.
Material and Methods: Between the relevant dates, 40 patients with COVID-19 disease who were operated for peripheral artery disease in our clinic were included in the study. Of the 40 samples, 20 were aortic tissue and 20 were endothelial tissue. These tissues consist of samples taken from the patient at the Cardiovascular Surgery Department. The samples were transferred to the Microbiology laboratory and extracted by PCR procedure. Laboratory diagnosis with the kit was performed by multiplex RT-qPCR targeting SARS-CoV-2 ORF1ab and N genes in a real-time PCR device.
Results: Eight out of 20 patients in the endothelial group were female and 12 were male. Ten out of 20 patients in the aortic group were female and 10 were male. All samples were found as negative by PCR for SARS-CoV-2 at two study groups (aortic and endothelial tissues).
Discussion: This valuable study was conducted under limited resources and challenging pandemic conditions. We believe that this study will serve as a preliminary investigation shedding light on the effects of atherosclerotic plaques in patients with COVID-19 and PCR positivity in long-term and large-scale cases.

Aim: Vitamin D is a fat-soluble steroid hormone that has a pivotal role in Calcium, Phosphorus and bone mineralization. It plays a crucial biological role in maintaining normal bone health, growth, and development, including teeth and jaws. This retrospective study was designed to explore the correlation between Vitamin D levels and the prevalence of apical periodontitis, extracted teeth, the size of apical periodontitis
Material and Methods: A total of 114 patients were obtained from the archives of patients who were previously recorded at the Faculty of Dentistry for dental diagnosis. Patients were classified according to their Vitamin D levels. All teeth were evaluated as missing and apical periodontitis. The Kolmogorov-Smirnov test was used to evaluate the normality. Due to non-normal distribution, the relationship between diagnosis and Vitamin D level was examined using the Kruskal-Wallis test. A model was constructed with linear regression analysis.
Results: No significant difference was found between Vitamin D levels regarding the prevalence of extracted teeth, apical lesion. Apical lesion size and Vitamin D level were negatively correlated.
Discussion: In this study, it was considered that Vitamin D deficiency negatively affects the size of apical periodontitis. However, it does not affect the prevalence of tooth loss and apical periodontitis.

Analysis of morphometric characteristics of aortic bifurcation: Computed tomography angiography study

Aim: The aim of this study was to determine the diameter and angle of the aortic bifurcation (AB) and its level in relation to the lumbar vertebra to provide useful information to surgeons interested in this region and contribute to the existing literature.
Material and Methods: Computed tomography angiography (CTA) images of 299 patients (156 females, 143 males) retrieved from Gaziantep University Hospital archives were reviewed. The long- and short-axis diameters of the AB were measured on two-dimensional images. The internal and external angles and the vertebral level of the AB were measured on three-dimensional reconstructions.
Results: The mean age of patients was 49.85 ± 18.74 years (range: 5 – 89). AB was most commonly observed at the middle L4 level (25.76%). The mean short-axis diameter of the AB was 1.29±0.26 cm, and the long-axis diameter of the AB was 1.92 ± 0.62 cm. Both diameter values were significantly greater in males than in females (p<0.05). The mean internal angle of the AB was 41.74±10.81°, and the mean external angle of the AB was 36.97±4.85°, with no significant difference between sexes (p>0.05). While the short-axis diameter of the AB and both internal and external angle values showed a very weak positive correlation with age, a moderate positive correlation was found between the long-axis diameter of the AB and age (0<r<0.2; p<0.05).
Discussion: Providing further insight into the morphology and morphometry of the AB, this study is expected to be useful in preoperative assessment and stent selection in anterior lumbar spine surgeries and endovascular aortic repair treatments.

Can TMT predict overall survival in patients with brain metastases from different primary malignancies as an indicator of sarcopenia?

Aim: The present study aimed to examine the accuracy of temporal muscle thickness (TMT) in predicting overall survival (OS) of brain metastases from different primary malignancies utilizing a publicly available database.
Material and Method: A total of 75 patients with metastatic brain tumors with different primary malignancies, whose data were obtained from open data sets, were included in the study. TMT was analyzed on axial thin section postcontrast T1-weighted images. Median TMT MMT was employed to establish the cut-off point.
Results: Evaluation of the median TMT measure (7.47 mm) served as the basis for establishing a survival threshold. Median overall survival was higher in the group with greater muscle thickness for TMT value (TMT<7.47 mm: 608 days TMT>7.47: 919 days). According to these results, there was no significant difference between the groups. Cox regression analysis indicated that TMT values less than median muscle thickness were negatively associated with overall survival (TMT<7.47: HR 1.63 CI 1.01-2.64, p = 0.044).
Discussion: We suggest that TMT is a promising marker for predicting the survival in patients with metastatic brain tumors with different primaries.

Aim: Patients with inflammatory bowel disease (IBD) are at a high risk of sarcopenia. This study aimed to evaluate sarcopenia by estimating muscle mass with magnetic resonance imaging of newly diagnosed IBD.
Material and Methods: Pediatric IBD patients, who underwent magnetic resonance enterography (MRE) from 2019 to 2022, were enrolled. Demographic, anthropometric, laboratory and clinical data were retrospectively obtained from medical records. The total Psoas Muscle Area (tPMA) and Skeletal Muscle Area (SMA) were calculated by scanning the psoas muscle areas at the upper level of the L4 vertebra and all muscle areas in the same section on axial T2-weighted images. Sarcopenia diagnosis was made according to Pediatric Reference Analytic Morphomics Population (PRAMP) growth charts.
Results: We enrolled 26 IBD patients, 11 (42.3%) with Crohn’s disease (CD) and 15 (57.7%) with ulcerative colitis (UC) (median age 15, IQR=4.25 years). Among patients, 14 (54%) cases had sarcopenia. IBD patients treated with biologics had a lower tPMA index (p=0.036) than those treated with 5-ASA and steroids. tPMA and tPMA indexes were significantly lower in the group with vitamin D levels below 20 ng/ml (p=0.018, p=0.015, respectively). A significant correlation was also found between the tPMA index and serum vitamin D levels (r=0.429, p=0.029).
Discussion: This study demonstrated a high prevalence of sarcopenia with 54% and lower SMA in newly diagnosed pediatric IBD patients. Muscle mass measurement in MRE studies highlighted that sarcopenia should be considered in vitamin D deficiency and in the case of normal anthropometric measurements.

Aim: In this study, our aim was to evaluate the relationship between the use of glucocorticoid (GC) therapy at any stage of Rheumatoid Arthritis (RA) and disease activity, functional status, and comorbidities.
Material and Methods: Our study, included 194 patients followed up with the diagnosis of RA. Demographic characteristics of the patients, duration and dose of GC use, laboratory results, general health perception, disease activity score-28 (DAS-28) and modified Health Assessment Questionnaire (m-HAQ) values were recorded by scanning their files. The Ppatients were divided into 3 groups: as those who currently use using GC, those who had used using GC in the past, and those had never used never using GC. The relationship between GC use and assessment parameters was investigated between the three groups.
Results: General health perception was better in GC users. M-HAQ value was worse in GC users. A positive correlation was observed between the use of GC and the presence of additional diseases, the occurrence of sensitive joints, the patient’s general health perception, and m-HAQ, while a negative correlation was noted with DAS-28. Additionally, the regression analysis revealed that the use of GC was associated with an increase in the occurrence of additional diseases, a rise in the number of sensitive joints and m-HAQ values, and a decrease in the DAS-28 value.
Discussion: While the use of glucocorticoids in patients with rheumatoid arthritis reduces disease activity, it might have a negative impact on the individual’s functional status and increase the risk of additional diseases. We believe that the potential adverse effects attributed to glucocorticoids can be reduced by following guidelines and considering patient-specific factors before and throughout the treatment course.

Aim: Pregnancy of adolescents is associated with numerous complications both for mother and newborns. This study sought to examine adolescent pregnancy in comparison with adult pregnancies in terms of maternal and fetal outcomes in a large sample size of healthy women.
Material and Methods: This retrospective study included data on 1,331 adolescents who had deliveries at the Department of Obstetrics. Data included maternal age, parity, gestational age, APGAR scores, maternal blood levels of thyroid stimulating hormone (TSH), triiodothyronine (T3), thyroxine (T4), and hemoglobin at delivery, birth weight, birth height, head circumference, and admission to the neonatal intensive care unit (NICU).
Results: Of 14,184 participating mothers, 1331 were adolescent mothers, and 12853 were controls. Adolescent mothers significantly differed from the control group with respect to three main domains of study: maternal, laboratory, and newborn. Significant maternal characteristics were delivery week (38.4±1.1 vs 39.7±1.1, p=0,0001), and higher incidences of preterm birth (7.0% vs 3.4%, p=0.032) and operative vaginal delivery (3.2% vs 2.6, p=0.006). Significant maternal laboratory findings were increased platelet count and increased TSH. Significant newborn characteristics were decreased weight (3,114±424 vs 3,245±439, p=0.0001), height (50.0±2.3 vs 50.4±2.2, p=0.0001), and head circumference (33.9±1.4 vs 34.3±1.3, p=0.0001), but higher incidences of LBW and stillbirth.
Discussion: Adolescent pregnancy and delivery is actually a public health problem, causing physical and psychological problems particularly for mothers, postpartum problems for the newborns, for their families, and social problems for the whole society.

Aim: The aim of the study was to evaluate patients who were followed up with lymphadenopathy and underwent biopsy in Afyon Health Sciences University Pediatric Hematology/Oncology Department and to assess the potential correlations between the presence of antibodies against EBV antigens in serum samples and the presence of EBER in lymph node tissues.
Material and Methods: All cases diagnosed with lymphadenopathy and excisional biopsy performed in the Pediatric Hematology and Oncology Department, between January 2018 to January 2020 were retrospectively evaluated. Medical records (gender, age, medical history, sore throat, fever, cough, history of upper respiratory tract infection, animal contact and travel, duration of symptoms) of patients, as well as their treatment history were examined. Tissue EBER status, serological tests for EBV (EBV VCA IgG, VCA IgM, EBNA IgG, EBNA IgM, and EA IgG) data were recorded from files.
Results: Our study included 22 patients comprising 10 females (%45.4) and 12 males (%54.5). At the time of the diagnosis, the median age was 8 (range, 2 to 17) years. Five (22.6%) of 22 biopsies were reported to be malignant. Reactive lymphoid hyperplasia was seen in 15 (68.1%) patients. Statistical analysis revealed a significant correlation between anti-VCA IgG serology and the EBER status (p=0.03).
Discussion: In conclusion, lymphadenopathy is a common clinical condition in childhood and often develops due to reactive hyperplasia. It was determined that patients with serum EBVVCA IgG positive were also EBER positive in the tissue.

Aim: The aim of this study is to identify the risk variables for mortality in patients undergoing venoarterial extracorporeal membrane oxygenation (VA ECMO) in order to better predict their prognosis.
Material and Methods: Following a retrospective analysis of VA ECMO patients treated between January 2013 and April 2021, they were classified into mortality and survival groups. Those over the age of 18 and on VA ECMO for more than 24 hours were included in the study.
Results: A total of 50 patients were included in the study, and the mortality rate was found to be 84%. VA ECMO support lasted an average of 5.8± 4.61 (1-18) days, while ICU stays averaged 17.8± 25.85 (1-124) days. (p < 0.05). The MELD-XI score (modeling end-stage liver disease without INR), the ECMO-ACCEPTS score (a VA ECMO scoring system for patients undergoing ECMO), the 18th and 24th lactate levels, the lactate peak value within the first 24 hours, following ECMO were all factors associated with mortality.
Discussion: The MELD-XI score, calculated during the early period, and the measurements of lactate levels at 12 hours, 18 hours, and 24 hours, and the peak lactate value, rather than lactate clearance, were identified as crucial prognostic factors for mortality. Furthermore, the ECMO-ACCEPTS score was also determined to be a significant predictor of mortality.

Aim: The purpose of this study was to evaluate the stiffness and volume of the testes in adult patients with varicocele using Doppler ultrasonography (DUS) and magnetic resonance elastography (MR-E), and to compare the findings to those from the patients with normal contralateral testes and those from healthy controls.
Material and Methods: A prospective comparison research with 46 varicocele patients (92 testes) and 46 control patients (92 testes) was ethically authorized. There were 92 healthy control testes in Group III, 40 healthy contralateral testes in Group II, and a total of 52 testes with varicocele in Group I. Group comparisons were made using one-way analysis of variance (ANOVA), and binary comparisons were made using Student’s t-test. Using Pearson’s correlation test, the relationship between testicular stiffness and volume was investigated.
Results: The mean MR-E stiffness values did not significantly differ between the two groups or the three groups (P 0.05). The mean testicular volume varied significantly between groups I and III (P = 0.022). The difference between groups I and II (P = 0.716) and groups II and III (P = 0.074) did not, however, differ significantly. In any group, there was no statistically significant relationship between testicular volume and stiffness.
Discussion: Testicular volume or varicocele was not significantly correlated with MR-E readings. The accuracy of MR-E in predicting testicular parenchymal injury requires further research with larger patient groups.

Aim: Mandibular condylar process fractures are the most common fractures in the maxillofacial region. Surgical treatment of condylar region fractures with miniplates and miniscrews has become more popular because it is a stable method that allows immediate function. The present study examined sheep hemimandibular subcondylar fracture models to evaluate the biomechanical properties of two different miniplate/screw system types used for surgical fixation of subcondylar fractures.
Material and Methods: Experimental standardized subcondylar fracture lines were examined in ten sheep hemimandibula models. Each segment was fixed with one of two fixation types: double straight miniplates with eight miniscrews or a single rhombic-shaped miniplate with five miniscrews. All models were mounted in a servohydraulic testing unit, and a continuous linear force was applied. Maximum force and displacement values were evaluated and statistically analyzed.
Results: The rhombic-shaped miniplate group had statistically significantly lower values than the double straight miniplate group for the maximum force, work at maximum load, and hardness. There was no statistically significant difference between the two groups in terms of the displacement at the maximum load.
Discussion: Considering the advantages of rhomboid mini-plates, further clinical and mechanical studies are needed for their use in the surgical fixation of subcondylar fractures.

Aim: In this study, our aim was to evaluate the predictive value of presepsin (P-SEP) for neonatal sepsis in pregnant women with preterm premature rupture of the membranes (PPROM).
Material and Methods: The study included 20 pregnant women between the ages of 18 and 40 who presented with PPROM between the 24th and 37th weeks of pregnancy. The control group consisted of a total of 40 pregnant women, confirmed by last menstrual period and early ultrasound examinations, between the 24th and 37th weeks of pregnancy. In our study, samples were collected from both maternal and fetal umbilical cords in the PPROM group and from maternal and fetal cord blood in the control group. Presepsin levels were analyzed in the laboratory using an ELISA kit.
Results: No statistically significant relationship was found between maternal presepsin, fetal presepsin levels, and clinical data and laboratory assessments of the pregnant women.
Discussion: There is no relationship between presepsin levels and neonatal sepsis.

Aim: The study aims to investigate the antiepileptic effect and toxicity values of Δ⁹-Tetrahydrocannabivarin, one of the major phytocannabinoids of the Cannabis sativa plant, by in silico methods.
Material and Methods: In this study, the molecular properties of Δ⁹-Tetrahydrocannabivarin obtained from the PubChem database were investigated using the SwissADME database. The toxicity class and Lipinski parameter of Δ⁹-Tetrahydrocannabivarin were estimated by Protox II, Toxtree, and SwissADME tools, respectively. Finally, the target proteins were determined to examine the effects of Δ⁹-Tetrahydrocannabivar on epilepsy; Voltage-gated sodium channels (VGSCs/NavMs) receptor, γ-aminobutyric acid type A (GABA A) and γ-aminobutyric acid aminotransferase (GABA-AT) were obtained from the Protein Data Bank and molecular docking was performed using AutoDock Vina 1.5.7. The docking process was visualized using Discovery Studio Visualizer 2021 and LigPlot V.2.2.8 imaging programs.
Results: Cannabis sativa has low toxicity, and has good binding energy; thus, it can be a suitable candidate for epilepsy treatment.
Discussion: Although Δ⁹-Tetrahydrocannabivarin can be a suitable candidate for epilepsy treatment, in vitro and in vivo studies are needed.

Aim: Carotid artery stenosis is the pathophysiological source of strokes and is an occlusive disease of the carotid artery. Stroke is a disease with high mortality and morbidity. Carotid artery stenosis (CAS) is one of the most important causes of ischemic stroke, and inflammation plays an important role in stroke and carotid atherosclerosis. Many studies conducted in recent years have shown that blood parameters can predict disease progression and mortality in diseases such as stroke and coronary artery disease. Within the framework of this main objective, the relationship between CAS and various hematological parameters in stroke patients was analyzed.
Material and Methods: This study was designed as a monocentric study, in which evaluated demographic and hematological parameters were evaluated in theat Fiırat University Faculty of Medicine, Department of Neurology in Elazig, in Turkey. The sample of the study included 84 participants (CAS Group: 37 and Control Group: 47) who were treated for carotid artery stenosis between September 2018 and March 2020 and were found eligible for suitable with patient inclusion criteria of the study. The age and body mass index of the research sample were recorded, and routine complete blood counts were evaluated.
Results: According to the results, there is not any difference between CAS and control in terms of demographic and clinical futures. Another result is that the mean Red cell distribution width (%), Neutrophil and Neutrophil/lymphocyte ratio of CAS group was significantly higher than in the control group, while the Mean corpuscular volume, Lymphocyte, Eosinophils, and Monocytes measures for the control group were significantly higher than in CAS group.
Discussion: The study results suggest that it may be a cheap and useful biomarker in predicting the development of Transient Ischemic Attack (TIA) in patients with stenting of the carotid arteries, since the ratio of neutrophils and lymphocytes in patients with stenting of the carotid artery and developed TIA differ significantly from hematological parameters.

Aim: On February 6, 2023, the province of Maraş province, in southern Turkey, was shaken by two destructive earthquakes measuring 7.7 and 7.6 degrees of the Richter scale, 8 hours apart. In earthquakes, peripheral nerve injuries (PNIs) frequently occur and cause significant long-term disability in surviving patients. This study aimed to analyzes the clinical, demographic, and electromyographic features of PNIs resulting from the Marashş earthquakes.
Material and Methods: The first electrophysiological test results of 130 earthquake victims were recorded. Demographic and clinical characteristics of the patients, electrophysiological features of PNIs detected in each patient, and other accompanying musculoskeletal injuries were recorded.
Results: The frequency of injuries was 98 (75.4%) peripheral nerves, 11 neural plexuses, four cervical radiculopathies, eight cauda equina lesions, three cranial neuropathies, and five entrapment neuropathies. The most frequently injured nerve was the sciatic nerve (46.2%); the second was the ulnar nerve (26.9%), followed by the median (18%) and radial nerves (18%). Combined PNIs were present in 36 (27.7%) patients.
Discussion: PNIs are one of the most frequent injuries in earthquake victims. Prolonged compression, soft tissue injuries, bone fractures, compartment and Crush syndrome, and exposure of extremities to traction during rescueing have been observed as facilitating factors in earthquake-related PNIs.

Aim: There are limited data specifically evaluating the relationship between allergic rhinitis (AR) and dyslipidemia, as well as obesity in children. In this study, we aimed to evaluate the relationship between AR, dyslipidemia, and obesity.
Material and Methods: A total of 43 patients diagnosed with AR and 35 healthy children were included in the study. Laboratory records, including total cholesterol (TC), triglyceride (TG), low-density lipoproteins-cholesterol (LDL-C), high-density lipoproteins-cholesterol (HDL-C) and very low-density lipoprotein cholesterol (VLDL-C) levels, were examined. Cases were assessed in terms of body mass index (BMI). Patients with AR and normal weight were classified as Group 1 (G1), patients with AR and obesity were classified as Group 2 (G2), control group with normal weight was classified as Group 3 (G3), and control group with obesity was classified as Group 4 (G4).
Results: The mean BMI in the patient and control groups was 21.6±5.4 and 22.2±6.1 kg/m², respectively (p: 0.394). There was no statistically significant difference observed in the serum levels of TC, TG, LDL-C, HDL-C, and VLDL-C between the patient and control groups, G1 and G3, as well as between G2 and G4. A negative correlation was found between serum total immunoglobulin E (IgE) levels and TC and LDL-C values. No correlation was found between eosinophil levels, AR severity, AR persistence, and lipid profiles.
Discussion: No significant relationship was found between AR and dyslipidemia or obesity. A negative correlation was found between serum IgE levels and TC and LDL-C values.

Aim: Recent studies showed that the immune inflammatory response plays a significant role in patients with asthma. The objective of the current study is to examine whether hemoglobin, albumin, lymphocyte, and platelet (HALP) score and systemic inflammatory response index (SII) are effective in predicting asthma.
Material and Methods: In this retrospective case-control study, seventy patients diagnosed with asthma and 70 healthy subjects followed in the Allergy and Immunology Clinics of Ordu University Education and Research Hospital were included. The HALP score was calculated according to the following formula: HALP score = (hemoglobin (g/L) × albumin (g/L) × lymphocytes (/L))/platelets (/L). The SII index was calculated according to this formula SII (neutrophil x platelet/lymphocyte count).
Results: Neutrophil, lymphocyte , monocyte, mean platelet volume (MPV), CRP were statitically significant betweeen the groups (p<0.05, Table 2). However, there was no statistically significant difference between white blood cells and platelets. A statistically significant difference was found between SII, HALP score neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte (PLR) indices between the study groups (p<0.05).
Discussion: We indicated that SII and and HALP scores may be cost-effective and easily accessible biomarkers in predicting asthma and both could be used as treatment response markers.

Aim: Non-small cell lung cancer (NSCLC) is a significant cause of cancer-related morbidity and mortality worldwide. In recent years, immunotherapies such as Nivolumab have revolutionized treatment strategies, demonstrating enhanced survival and response profiles compared to traditional platinum-based therapies. This study elucidated the prognostic significance of pretreatment hematological parameters, including Neutrophil- Lymphocyte Ratio (NLR), Systemic Immune-Inflammation Index (SII), and Prognostic Nutritional Index (PNI), in NSCLC patients undergoing Nivolumab treatment.
Material and Methods: This retrospective study included patients treated with Nivolumab for pathologically proven advanced NSCLC between January 2019 and March 2021. Hemogram parameters and clinicopathological and clinicopathological factors before Nivolumab treatment were recorded from the hospital’s electronic data record system. Survival analysis was conducted using the Kaplan-Meier method and the Log-Rank test was utilized for comparison. Multivariate analysis of factors affecting survival was performed with the Cox proportional-hazards model.
Results: Ninety-two patients were included in this study. The median OS and PFS for all patients included in the analysis were 14.5 months and 11.1 months, respectively. The ideal cut-off value dividing NLR into 2 for overall survival time was calculated as 1.89 (AUC: 0.694, p=0.002) using Roc-Curve. In univariate analysis, ECOG performance score (p<0.001), leukocytes (p < 0.029), high neutrophil (p < 0.018) as well as NLR (p <0 .018) were found to be associated with worse overall survival. Multivariate Cox regression model demonstrated that ECOG (HR=3.54, 95% CI, 1.46-8.60, p =0.005) and NLR (HR=1.52, CI 1.16 – 2.01, p=.003) were significant predictors factors for survival.
Discussion: These results underline the critical role of inflammation and immune response in tumor progression and therapeutic outcomes. The identified markers offer a practical means of predicting patient responses to Nivolumab therapy, aiding in personalized treatment decisions. As NSCLC treatment paradigms shift towards immunotherapy, these easily accessible parameters could aid clinicians in selecting optimal therapeutic strategies for individual patients, ultimately contributing to improved clinical management and outcomes.

Aim: Infection causes morbidity and mortality in the elderly population, especially in intensive care units. Increasing age causes multiple co-morbidities, immune system, and multiorgan disturbances, and frailty. Elder population is prone to infections. There are little data about the impact of infection on short and long-term mortality in coronary intensive care units in elderly cases.
Material and Methods: Data from 466 patients aged 70 years and older admitted to the coronary intensive care unit between January 2010 and December 2014 were retrospectively reviewed.
Results: Patients with infection had higher in-hospital and 30-day mortality rates. There was no difference between groups regarding long-term mortality. The presence of infection was found to be an independent predictor of in-hospital mortality. The occurrence of infection was not associated with long-term mortality. In multivariate Cox regression analysis, age, previous CHF, and ACS type were independent predictors of long-term mortality.
Discussion: Infection was independently related to hospital mortality in the geriatric group in the coronary intensive care unit but had no impact on long-term mortality.

Aim: Polycystic ovary syndrome (PCOS) is a heterogeneous condition characterized by menstrual irregularities, hyperandrogenism, obesity, hirsutism, and psychological and psychosexual morbidity. This study aims to investigate the relationship between clinical, anthropometric, laboratory and ultrasonography findings in adolescents with the diagnosis of PCOS.
Material and Methods: Forty-one adolescent girls who were admitted to the Pediatric Endocrinology Clinic of Adana City Training and Research Hospital between January 2020 and December 2021 and diagnosed with PCOS according to the Rotterdam criteria were included in this retrospective cross-sectional study. Demographic data, hormonal and biochemical analysis of the patients were obtained from the files.
Results: The mean age of admission of the patients was 15.62±1.43 years, and the mean age at first menarche was 11.98±1.32 years. Oligomenorrhea (90.2%) was the most common admission complaint. Comparing the groups with PCOS with and without hirsutism, a significant difference was found in inhibin-B level (66±45 ng/dl vs 107±37 ng/dl; p=0.038). When comparing the groups with and without ultrasound findings compatible with PCOS, a significant difference was found in AMH level. (11.0±7.3 mcg/L vs 5.9±2.8 mcg/L; p=0.032). When overweight or obese patients with PCOS were compared with the normal weight group, the difference was significant in HOMA-IR (5.0±6.2 vs 1.5±0.65; p=0.009) and first menstrual age (11.3±1.3 years vs 12.7±0.75 years; p=0.008).
Discussion: In this study, while low inhibin-B was more common in patients with PCOS with hirsutism, AMH was found to be significantly higher in the group whose ultrasound findings were compatible with PCOS. These parameters can be investigated in the diagnosis of adolescent PCOS.

Aim: This study aimed to examine the impact of maternal BMI on embryo quality, euploidy rates, and embryo morphokinetics in recurrent pregnancy loss (RPL) patients.
Material and Methods: We conducted a retrospective analysis of 98 RPL patients and 403 embryos observed using the Time-Lapse Monitoring System (TLM) at Memorial Sisli Hospital, ART, and Genetics Center between April 2021 and November 2022. Patients were categorized into three BMI groups: Group 1 (<25 kg/m²), Group 2 (25-29.9 kg/m²), and Group 3 (≥30 kg/m²). All embryos underwent assessment for quality, ploidy, and morphokinetic characteristics based on BMI. Morphokinetic analysis was performed on euploid embryos (n=168) to eliminate aneuploidy’s impact on development dynamics.
Results: Age, Anti-Müllerian hormone (AMH), infertility duration, and previous IVF cycles were similar across groups. Group 2 had a slightly longer ovarian stimulation duration than Group 1 (p=0.040). IVF cycle outcomes, including oocyte retrieval, mature oocytes, and 2PN embryos, were similar. There were no significant differences in euploid embryo numbers among BMI groups. The distribution of good-quality and top-quality embryos was also similar (Group 1: 90.7%, Group 2: 87.7%, Group 3: 87.9%). Maternal BMI significantly affected the time embryos reached the expanded blastocyst stage (tEB) among the groups (p=0.044).
Discussion: This study found that high BMI does not impact embryo ploidy and quality but may prolong the morphokinetic process, specifically the time from morula to blastulation and the attainment of an expanded blastocyst, in cases of recurrent pregnancy loss (RPL).

Aim: Pulmonary embolism (PE) is a disease with a high mortality rate that occurs as a result of the migration of thrombus in the deep veins of the lower extremities into the pulmonary circulation. Contrast-enhanced CT and V/P scintigraphy are two commonly used methods for imaging. This study aims to determine the frequency of parenchymal and non-parenchymal findings on non-contrast CT in patients who are clinically suspected to have PE but who cannot undergo contrast-enhanced CT-angiography and who are diagnosed with PE by lung perfusion scintigraphy.
Material and Methods: Thirty-nine patients who were admitted to Tokat State Hospital between 2015 and 2022 with a preliminary diagnosis of pulmonary embolism and who underwent lung perfusion scintigraphy and non-contrast CT scans in the same week were evaluated retrospectively.
Results: When computed tomography findings were evaluated in patients with pulmonary embolism, wedge-shaped opacity was present in 3 patients (20%) but not present in 12 patients (80%); consolidation was present in 6 patients (40%) and absent in 9 patients (60%); dilatation in the main pulmonary artery was present in 7 patients (46.7%) and absent in 8 patients (53.3%).Ground-glass opacity was present in 11 patients (73.3%), absent in 4 patients (26.7%); pleural effusion was present in 4 patients (26.7%) but not in 11 patients (73.3%); atelectasis was present in 4 patients (26.7%) but not in 11 patients (73.3%).
Discussion: In our study, there was no statistically significant difference between the presence of pulmonary embolism in lung perfusion scintigraphy and the detection of wedge-shaped opacity, consolidation, dilatation in the main pulmonary artery, ground glass attenuation, pleural effusion and atelectasis in thorax CT.

Aim: Root canal localization is necessary for proper canal disinfection and obturation. This study aimed to examine the missing canals in terms of aging in endodontically treated 1st and 2nd molars in a Turkish subpopulation.
Material and Methods: The study included maxillary and mandibular molar teeth of 619 subjects (317 female and 302 male). Cone-beam computed tomography images of 490 molar teeth representing at least one missing canal were divided into 3 groups according to the age of the subjects: 18–44, 45–64, and more than 65. The data were evaluated with the chi-square test, with the significance level set at a p-value of 0.05.
Results: The incidence of missing 2nd mesiobuccal canals in the 18-44 age group and 1st mesiobuccal canals in the 45-64 and over 65 age groups were statistically higher than in the other age groups ( p<0.05). The incidence of missing 2nd mesiobuccal canals in maxillary 1st molars was statistically higher than in maxillary 2nd molars (p<0.05). There was no statistical difference between the types of missing canals and age groups in mandibular molars (p=0.772).
Discussion: Missing canals were more frequent in the mesial roots of mandibular molars with aging. With age, the incidence of missing canals increased in maxillary first molars but decreased in maxillary second molars.

Aim: Cataract and osteoarthritis are important diseases that cause morbidity in the geriatric population. Age is an important risk factor for both diseases. There is no clear information about the correlation of the severity of diseases yet.
Material and Methods: The cataract score for the right and left eyes and the osteoarthritis stage for the right and left knee of each patient were determined. After staging, body awareness questionnaire (BAQ) was administered to the patients to test their body awareness. In the study, the correlation of the patients’ cataract scores and osteoarthritis stages was examined. In addition, relationships between the cataract score and the BAQ score and between the osteoarthritis stage and the BAQ score were analyzed.
Results: There was a significant correlation between patients’ cataract scores and osteoarthritis stages, but this correlation was not strong. Cataract progression was found to be more asymmetrical than osteoarthritis. It was also found that the relationship between the cataract stage and body awareness was much stronger than the osteoarthritis stage.
Discussion:The diseases identified in this study have the potential to cause significant clinical and economic consequences when inadequately treated. Therefore, geriatric screening programs can provide a chance for treatment before diseases progress.

Aim: Upper gastrointestinal endoscopy is commonly performed as an outpatient procedure, which may lead to overlooking potential arrhythmias. In this study, we aimed to investigate the effects of intravenous or topical lidocaine on hemodynamics, QT interval and throat pain in patients scheduled for upper gastrointestinal endoscopy.
Material and Methods: The patients were randomly divided into three groups: Group I received 1 mg/kg IV(ıntravenous) propofol induction, Group II received 1 mg/kg IV propofol induction and topical lidocaine (9 sprays, 3 sprays at 10-second intervals, totaling 90 mg), and Group III received 1 mg/kg IV propofol induction and 1.5 mg/kg IV lidocaine induction. ECGs(Electrocardiogram) were obtained before and after the procedure, and hemodynamic data were recorded. Throat pain in patients was assessed after the procedure.
Results: There was a statistically significant difference in systolic blood pressure (SBP) values at the 3rd minute among the three groups (p=0.021). The SBP values at the 3rd and 5th minutes in Group I were significantly lower compared to Groups II and III (p=0.021, p=0.012 retrospectively). There was a statistically significant difference in diastolic blood pressure (DBP) values among the three measurements in Group I (p=0.0001). The DBP values at 1 minute in Group I were significantly higher compared to the values at the 3rd and 5th minutes. Additionally, there was a statistically significant difference in postoperative QTc values among the groups (p=0.001).
Discussion: We concluded that 1.5 mg/kg IV lidocaine effectively suppressed the hemodynamic response secondary to adrenergic activation during upper gastrointestinal endoscopy and also suppressed the increase in QT and QTc values.

Aim: This study aimed to observe the effects of the conservative treatment, locking plate, hook plate and tension band fixation techniques used for distal clavicle fractures (DCF) on fracture healing, pain, functional outcomes and complications.
Material and Methods: Ninety-four patients treated conservatively and surgically for DCF between 2016 and 2020 were included in the study. Age, gender, Neer classification of the fracture, treatment type, fracture side, fracture healing period, Constant shoulder outcome score, Visual Analog Scale (VAS) scores and complications were examined. Statistical analysis findings were evaluated with the IBM SPSS 28 program at p<0.05 significance level.
Results: Of the patients, 77 % were male and 23% were female. 51% of individuals were under 40 years of age. The median follow-up time was 13 months. Conservative treatment was applied to 34% of the individuals, locking plate to 36.2%, hook plate to 14.9% and tension band wiring to 14.9%. There was a moderate negative correlation between the VAS Score and the Constant Score (p<0.05). There was no statistically significant difference between the groups in terms of Constant scores, VAS scores and fracture healing (p>0.05). Complications were seen in the hook plate and tension band wiring groups, but not in the locking plate group.
Discussion: There is no optimal treatment method for DCFs. In this study, no significant difference was found between methods. The complication rate with the locking plate method is lower than with other surgical methods.

Aim: Studies on the treatments and their effectiveness in Wernicke’s Aphasia are quite limited. The development of technology and the use of virtual reality implementations in rehabilitation processes increase the importance of studies reporting the effectiveness of the treatment in this regard. The aim of the study was to show the effectiveness of Virtual Reality-Assisted Language Treatment (VR-Assisted SLT) implementations in post-stroke Wernicke’s Aphasia.
Material and Methods: In this context, 30 people were included in this double-blind study that had a complete experimental design and consisted of SLT, Control Group and VR-Assisted SLT who underwent the VR-Assisted treatment process by receiving the NeuroVR 2.0 open source software as a model. Ten participants were assigned for each group. Those receiving treatments received 5 sessions (minimum 45 minutes) weekly for 4 weeks. ALAT and SAQOL-39 were applied as pre-test, post-test, and follow-up tests.
Results: Naming and auditory comprehension scores were found to increase significantly over time, especially in the VR-Assisted SLT Group. Although similar increases were detected in the quality of life scores in the SLT and VR-Assisted SLT Groups, the VR-Assisted SLT Group had higher scores on physical and psychosocial subscales of the quality of life than the Control Group (p<0.05).
Discussion: VR-Assisted SLT applications can enhance language and quality of life scores by providing realistic experiences.

Aim: Systemic immune-inflammation index (SII) has been used as a marker in several diseases, but its association with ovarian torsion is unknown. The aim of this study was to determine the diagnostic value of SII preoperatively in patients with ovarian torsion.
Material and Methods: In this retrospective study, preoperative neutrophil/lymphocyte ratio (NLR) and SII values were compared in 124 patients with surgically confirmed ovarian torsion and 107 control subjects without ovarian torsion.
Results: NLR and SII were statistically higher in the torsion group compared to the control group. The predictive performance of SII for ovarian torsion (AUC = 0.788) was better than NLR (AUC = 0.665). The preoperative NLR and SII cut-off values were 2.48 and 794.04, respectively.
Discussion: High SII values can be used to support the surgical decision in patients with suspected torsion. SII is a better predictor of ovarian torsion than NLR.

Aim: The diagnosis, treatment, and follow-up of infantile tumors are of special importance among all pediatric malignancies. Here in this study, we aimed to evaluate the demographic, clinical, pathological, and surgical properties of infantile tumors followed by our department.
Material and Methods: We evaluated 78 cases of infantile tumors <12 months of age who were registered in Gazi University, Faculty of Medicine, Department of Pediatric Oncology between 1991-2021.
Results: Infantile tumors account for 6.8% of all pediatric malignancies followed by our department. There were 38 girls and 40 boys whose mean age was 186.60±110 days (8 -350 days). Nine of our patients were diagnosed during the neonatal period. The most common tumor was neuroblastoma in 24 patients (30.7%) while the second most common tumor was central system tumor in 13 cases (16.6%). Other tumors included 10 mature/ immature teratomas, 8 renal tumors (10.2%), 6 retinoblastomas (7.6%), 6 liver tumors (7.6%) and soft tissue sarcoma in 6 patients, yolk sac tumors (3.8%), and Langerhans cell histiocytosis (2.5%). Fifty-one patients (65.3%) received chemotherapy, 19.4% received radiotherapy and 86.6% had surgical resection for treatment modalities. Total surgical resection has been applied to 67.2% of patients. Relapses were observed in 23 patients (29.4%), most commonly at the site of the primary tumor. Forty-four patients are alive without disease, 9 patients were lost to follow-up, and 25 patients died. The follow-up period of the patients ranged from 4 months to15 years. Overall survival was 67% and event-free survival was 58%.
Discussion: In our study, neuroblastoma was the most common solid tumor seen in the infantile period, consistent with the literature. With intensive treatment modalities and awareness of treatment-related toxicities for solid tumors of the infantile period, long-term survival can be achieved.

Aim: In this study, we aimed to determine the nodule activity in thyroid scintigraphy in patients with thyroid nodules, as well as to determine the contribution of dynamic radionuclide thyroid Scintigraphy (RTS) in distinguishing between benign and malignant nodules.
Material and Methods: A total of 48 patients (30 women, 18 men) with hypo- and/or hyperactive thyroid nodules on thyroid scintigraphy larger than 1 cm were included in the study. Dynamic imaging was achieved in the thyroid region of the patients under a gamma camera and with an intravenous injection of 185 Mβq Tc99mpertechnetate. Retention rate (RR) was calculated using these curves and static anterior image counts. RR was found by drawing a total of 75 nodules of 48 patients. Power Doppler ultrasonography (PDUS) was performed on each patient at the same time and central peripheral resistive index (SRI) and peripheral resistive index (PRI) were measured. Nodule vascularization pattern was classified in PDUS.
Results: The mean age of the patients was 54.12 ± 13.41 years. While 40 patients had a fine needle aspiration biopsy (FNAB), 8 patients did not have an FNAB. Of the 64 nodules in 40 patients who underwent FNAB, 20 were identified as non-diagnostic, 38 as benign cytology (BC), and 6 as malignant cytology (MC). In the time activity curve obtained from RTA, it was determined that the probability of malignancy was high in nodules that were hyper-vascular and showed rapid clearance. While the average RR of nodules in BC was 0.41±0.70, it was calculated as 0.43±0.23 in MC. The average central RI in nodules in BC was 0.39±0.12, while in MC it was 0.30±0.63. The average peripheral RI value was calculated as 0.60±0.45 in BC and 0.62±0.29 in MC. Although our number of malignant cytology nodules was quite low, there was a statistically significant positive correlation between the malignant cytology RR value and the malignant cytology peripheral RI value (p = 0.003, r = 0.043). There was a significant positive correlation between the BC central RI value and the BC RR value (p = 0.012, r = 0.054).
Discussion: We think that dynamic RTS, which is a method that will increase the sensitivity of classical thyroid scintigraphy, support the necessity of FNAB, and will not bring additional radiation dose or difficulty to the patient, can be used routinely and has diagnostic value in the benign-malignant discrimination of thyroid nodules.

Aim: In this study, we aimed to evaluate the efficacy of vaginal micronized progesterone (VMP) therapy in the treatment of endometrial hyperplasia without atypia by comparing it with levonorgestrel intrauterine device (LNG-IUD) therapy.
Material and Methods: This retrospective study included 133 premenopausal females with endometrial hyperplasia without atypia. The patients were divided into two groups according to VMP (n = 68, 200 mg once a day, 10 days/cycle) and LNG-IUD (n = 65) treatments. The groups were compared in terms of endometrial thickness measurements, endometrial sampling results, and hemoglobin (Hb) and hematocrit (Hct) values before and after treatment.
Results: Regression rates (90.8% with LNG-IUS vs. 88.2% with VMP; p = 0.601) and endometrial thickness values (4.5 mm with LNG-IUS, 5 mm with VMP, p = 0.382) were similar between the groups. A significant increase was observed in the VMP group in terms of blood parameters (Hb, Hct) after treatment (p < 0.05).
Discussion: VMP is as effective as LNG-IUD in treating hyperplasia without atypia.

Aim: We aimed to evaluate risk categorization according to the Sydney classification in lymph node aspirations performed in our hospital, which has a large archive.
Material and Methods: Three hundred fifteen lymph node fine-needle aspiration (FNAB) smears were reassessed and classified using the Sydney classification. Clinical and histopathological findings were compared. Risk of malignancy (ROM), sensitivity, specificity, positive predictive value, negative predictive value, and accuracy were evaluated.
Results: Fine needle aspiration biopsies (FNAB) of a total of 315 cases, 74 (23.5%) men and 241 (76.5%) women, were examined. One hundred sixty-eight of 315 cases had a history of previously diagnosed tumors. In terms of diagnostic categories, 12 (3.8%) cases were L1 (non-diagnostic), 122 (38.7%) L2 (benign), 7 (2.2%) L3 (AUS/ALUS), 27 (8.6%) L4 (suspicion of malignancy), and 147 (46.7%) were in the L5 (malignant) category. Thus, L5 and L2 were the most frequently used categories. There was discordance in the histopathologic examination of 25 (20,5%) cases (mostly Hodgkin’s lymphoma) in the L2 (benign) category. The ROM in the L1 category was 58.3%, in the L2 category- 20.5%, in the L3 category- 57.1%, in the L4 category- 92.6% and in the L5 category- 100%. The diagnostic performance of lymph node fine-needle aspiration biopsy (FNAB) was evaluated by calculating sensitivity (98,1%), specificity (82,7%), positive predictive value (74,5%), negative predictive value (98,9%), and accuracy (87,9%).
Discussion: We believe that the Sydney classification for reporting FNAB improves the quality of the procedure, understanding of the report, and communication between clinicians and cytopathologists, leading to better patient management.

The stomach is the most common location of the heterotopic pancreas (HP). Most gastric lesions have benign behavior with mild or nonspecific symptoms. The limited number of malignant HP cases presenting with gastric outlet obstruction have been published. A 69-year-old man presented with epigastric distension, nausea, vomiting, and weight loss for two months. Upper gastrointestinal endoscopy revealed a dilated stomach and pyloric submucosal wall thickening causing complete outlet obstruction. Computed tomography demonstrated dilated gastric lumen with circumferential wall thickening in the pylorus. At laparotomy, the stomach was dilated, and a firm mass obstructing the lumen was palpated in the pyloric region. Subtotal gastrectomy with gastrojejunostomy was performed. Histopathologically, a submucosal adenocarcinoma arising from HP tissue was found. Although rare, gastric HP should be always kept in mind in patients presenting with pyloric mass causing gastric outlet obstruction.

Rectus sheath abscess is a rare condition with a high mortality and morbidity. They usually occur spontaneously, after trauma or surgery and located infraumbilically. Rectus sheath abscess is often misdiagnosed as acute abdomen or tumors of the abdomen. Correct diagnosis and treatment must carried out as soon as possible. Diagnosis is made using clinical, laboratory and radiological data. Ultrasonography and computed tomography significantly help in the correct diagnosis of rectus sheath abscesses. Incorrect diagnosis can cause unnecessary antibiotic treatments and negative laparotomies with a high rate of mortality and morbidity. A 27-year-old male patient was admitted to our polyclinic with fever and sweating for the last 3 months. There was no history of chronic disease, with the exception of surgery due to an inguinal hernia a year ago. The patient underwent abdominal computed tomography, and an abscess was detected on the rectus sheath. The abscess was drained and the patient was discharged from the hospital after the completion of the current antibiotic therapy. Here, we wanted to emphasize that rectus sheath abscess may be a late complication of inguinal hernia repair and evaluation should be done earlier with computed tomography considering surgical intervention history.
